I have a chunk of turquoise that is approximately 2-1/2" wide and 1-1/4" thick.
If possible, I would like to know what type it is and if it is from the Nevada or Arizona area.

Looks like turquoise mountain rough. You should be able to do a google search for turquoise mountain rough to get an idea of price, you might try http://www.colbaugh.net/ourproducts/roughkingmanturquoise.html to get some information.
